Dear partners!

In order to ensure due diligence when choosing a counterparty, LLC “VERTEX” requests you to provide duly certified copies of the following documents:

For legal entities:

1. Certificate of state registration of a legal entity / record sheet for entering information into the Unified State Register of Legal Entities.
2. Certificate of tax registration of a legal entity.
3. Charter (with all changes and additions, if any apply).
4. Decision / protocol on the election of the company’s head.
5. Order on the assumption of the position of the company’s head.
6. Power of attorney, if the contract is signed by a representative.
7. Documents confirming the legitimacy of the legal / actual address (lease / sublease agreement, certificate of ownership, etc.).
8. Counterparty company information with all bank details.
9. Order on the appointment of the chief accountant.
10. Annual balance sheet (Form No. 1) and Profit and Loss Statement (Form No. 2), VAT and profit declarations for the last reporting period with a mark on delivery to the tax authority.
11. Certificate of the average number of employees for the last reporting period with a mark on delivery to the tax authority.
12. Notification (application) of the transition to a special tax regime (in the case of a special tax regime).
13. Licenses, certificates, certificates, etc. (in the presence of activities requiring special permission).
14. Decision / Report on the approval of a major transaction, if the transaction for a legal entity is major.

For individual entrepreneurs:

1. Certificate of state registration of an individual as an individual entrepreneur / record sheet for entering information into the Unified State Register of Legal Entities.
2. Certificate of registration of an individual with a tax authority on the territory of the Russian Federation.
3. Passport of a citizen of the Russian Federation (sheets: with the date of issue, photograph, registration).
4. Notification (application) of the transition to a special tax regime (in the case of a special tax regime).
5. Licenses, certificates, certificates, etc. (subject to any types of activities requiring special permission).

For individuals:

1. Passport of a citizen of the Russian Federation (sheets: with the date of issue, photograph, registration).
2. Certificate of registration of an individual with a tax authority on the territory of the Russian Federation.
3. Insurance certificate of state retirement insurance.
